Getting Started with Home care
We make it easy for you to get started with home care.
- Contact us by telephone or email. A member of our management team will respond to you within 24 hours. We will discuss your needs and how we can support you.
- Next, we will schedule a time to meet with you in person. Your client care manager will meet with you to discuss your specific needs and to answer any questions you may have. This provides an opportunity for us to get to know one another and builds a foundation of trust and good communication.
- We will work together to create a care team and a personalized care plan that is right for you. Your plan may include personal care (bathing, grooming, dressing, etc.), emotional support, social and recreational activities, help with household tasks, transportation services to help you get to appointments and meetings, etc.
- Your personal and unique care team will ensure you have continuity of care and direct assess to your client care manager. We will proactively address your changing needs and respond with appropriate adjustments.
- We will be there for you every step of the way!
